Day Hospital Program
for Medical Rehabilitation
Miller Dwan's Rehabilitation Center exists to assist persons with activity limitations, of all ages, to achieve realistic functional goals and to reach their highest feasible level of functioning by providing the most comprehensive, specialized, intensive, interdisciplinary medical rehabilitation programs in the region.
Our vision is to champion and respond to the overall needs of people in our region who have needs for medical rehabilitation across the life span. To support this, the Rehabilitation Center offers a Day Hospital Program for patients requiring medical rehabilitation.

People who may be eligible
for
the Day Hospital Program include:

Persons not requiring 24-hour delivery of rehabilitation nursing or
daily monitoring of status and condition by a rehabilitation physician.

Any patient with an
activity limitation who has the ability to
improve in a goal directed program.

A person with a direct rehabilitation physician referral.

Persons living locally or temporarily residing locally as patients
go home each afternoon.

Patients who can take responsibility for their own transportation to
and from the Rehabilitation
Center.
Our Day Hospital Program fills the gap between traditional inpatient and outpatient medical rehabilitation. It is generally more intensive than outpatient therapy-providing multiple treatment sessions from several clinical services including additional hours of therapy, education, and any necessary nursing supervision during the course of day treatment. Patients and families should consult with their physician to determine if the Day Hospital program is an option for them. Parents or caregivers must accompany minor children, unless specific arrangements are made and appropriate.
In following our philosophy, this program is tailored to the person's unique and individual needs. It is goal directed, adhering to specific criteria.
Functional goals may include areas of self care, mobility, thinking and understanding, adjustment to disability, bowel & bladder control, self medication, or speech and language. For some patients, the Day Hospital program could be a bridge to community re-entry from a hospital. It may assist other patients to achieve goals that help them remain living in the community.

Program features:

Patients receive therapy from a treatment team that works
collaboratively to best help the patient meet his or her goal. These teams
focus on the individual needs of each patient, in all age groups, and
include a variety of therapists, nurses, and social workers.

Limited nursing assistance is provided as needed.

The attending rehabilitation physician sees patients
as their treatment plan indicates,
generally one time per week.

Patients and their family attend care
conferences with their team every two weeks to monitor and discuss treatment and goals.

Education for the patient and family enhances the individualized
treatment program.

The program's usual hours are from 8 AM to 4 PM, Monday through
Friday. However, hours can be individualized based on patient need and
recommendations from the treatment team.

Space is available for patients to rest between
therapies.

Patients participating in the Day Hospital
Program bring their own meal, or may purchase meals in the cafeteria. Simple meal preparation can
also be incorporated into therapy.

Medications needed during the day are
brought from home. Staff is available to supervise medications according to the prescription label.
Miller Dwan's Rehabilitation Center believes each individual is a total human being with unique and individual abilities and desires, and those individual abilities and desires, and family support, direct our planning and delivery of rehabilitation programming. Our programs are characterized by the use of skilled clinicians working and planning in an interdisciplinary fashion to achieve functional goals identified by the persons we serve. It is our philosophy to provide an environment promoting maximum levels of independence as well as optimal psychological, social, and personal adjustment for people with activity limitations and their families.
